The following forms are available here for you to print and fill out before your appointment. You will need to bring the completed Acknowledgement of Receipt of Notice of Privacy Practices, the Welcome To Our Office, and the Medical History Questionaire with you to your appointment. Please also try to arrive 10 minutes prior to your scheduled appointment time so we may prepare your chart and stay on time. The Records Release may be helpful if you want to request copies of your prior patient records. The Patient Survey can be completed after your first visit to our office.
